Hospice Team

Hospice requires a collaborative effort with an interdisciplinary team that works closely with the patient and their family. Every team member plays a crucial role in creating a comforting and effectice environment. If you or a loved one need hospice care, we are here to provide quality care in all necesssary aspects.

Our promise to you

At Sanctuary Hospice, our staff provides care to the patient and works with their family or caregivers to develop the skills they need to follow the patients hospice plan. In addition, you also have access to our aides 24/7 after-hours and on weekends. Our hospice care team delivers patient satisfaction in all they do.

Medical Director

Certify the terminal prognosis.

Assess patient needs, manage symptoms, prescribe treatment.

Lead interdisciplinary team in development of care plan.

R

Provide consultation to other physicians regarding hospice care.

Hospice Nursing

R

Assess patient and family needs.

R

Provide training and coaching to improve comfort and resolve distressing symptoms.

R

Develop a care plan to meet identified needs.

R

Coordinate care with physician, caregiver and hospice staff.

R

Coordinate team visits and ensure implementation of approved plan of care.

Social Worker

R

Assess emotional, social and financial needs of patient and family.

R

Addresses questions regarding services.

R

Assists with applying for and contacting community resources, Veteran programs, financial assistance and other appropriate services and programs.

R

Makes arrangements for respite admissions.

R

Provides counseling and supportive services.

Hospice Aid

R

Provide direct personal care and hygiene support to patient.

R

Reports identified needs and concerns to RN Case Manager.

Chaplain

R

Assess spiritual needs of patients and their families.

R

Assist with memorial preparations.

R

Collaborate with your chosen faith organization.

R

Addresses your questions about God, faith or other topics.

R

Supports the emotional and spiritual needs of patient and loved ones.

Bereavement Counselor

R

Provides grief counseling.

R

Conducts support groups and other resources.

R

Checks on the bereaved with phone calls and visits.

R

Provides information on loss and healing.

R

Offers bereavement support for 13 months following the death of a loved-one.

Volunteers

R

Provide companionship and support to patient and family.

R

Provided non-medical services and respite time for caregivers and family.
